Job Overview: The Case Manager will have the opportunity to join a growing, innovative company focused on creating opportunities and connections for adults with intellectual and/or developmental disabilities (I/DD) to experience rich, meaningful lives and futures.

Responsibilities and Duties:

  • Manage a caseload of clients; including but not limited to:
  • Partnering with the onboarding team, supervisor, family, and adult client to build and implement a customized plan of support.
  • Partnering with the onboarding team and supervisor to onboard new clients to the caseload
  • Providing direct case management support to the adult client and their family including:
  • Visiting the client in-person weekly (at their home and/or out in the community)
  • Building and/or strengthening community connections (programming, friendships, relationships) and attending in-person events or social hang-outs when appropriate
  • Building, growing, and sustaining the client’s team of support; family, friends, staff, professionals and natural support
  • Identifying and applying to relevant employment, internship, and/or volunteer opportunities
  • Providing hands-on coaching in executive functioning, social skills, and life skills (e.g. role-playing social scenarios with the client, building a calendar with the client focused on interests and purpose, or cooking with the client)
  • Advocating for client’s medical needs and coordinate and join clients at doctor’s appointments
  • Supporting the family to maximize state benefits, including applying to OPWDD, attending life plan meetings, and manage reimbursements with fiscal intermediaries
  • Supporting the family in exploring housing options, roommate matching and/or Live in Caregiver matching
  • Providing parent and family coaching / mediation
  • Offering partnership and guidance in regards to future planning
  • Other responsibilities include:
  • Communicate clearly and effectively with clients and their families in an ongoing manner
  • Write proficient case notes and provide thorough documentation
  • Attend individual and group supervision
  • Attend professional development training sessions

Qualifications and Characteristics of the Ideal Candidate:

  • Masters Degree in Social Work, Mental Health Counseling, Occupational Therapy, Speech-Language Pathology or other related fields
  • LCSW supervision provided for clinical hours
  • Passion for working with/advocating for adults with I/DD and their families
  • Thinks creatively when making connections and maximizing partnerships
  • Belief in a person-centered approach that promotes a dignified and meaningful life for people of all abilities
  • A background in state and federal benefits and entitlements preferred (OPWDD, Self Direction, Medicaid, Medicare, SSI and SSDI)
  • Experience in supportive and/or affordable housing a plus
  • Ability to develop relationships with professionals in the field and create new opportunities for adults with I/DD
  • Strong written and verbal communication skills

Job Type : Full-time

Pay : $65,000.00 - $80,000.00 per year
