Job Details & Responsibilities:

• Conduct clinical reviews of pharmacotherapy, including drug interactions, side effects, and administration support.
• Document therapy reviews to inform nursing follow-up assessments.
• Counsel patients on new drug therapies based on clinical protocols.
• Coordinate care with referring clinicians, addressing patient-specific needs and documenting custom services.
• Act as liaison among patients, caregivers, nurses, and prescribers to ensure coordinated care.
• Provide subcutaneous injection training and arrange nursing support when needed.
• Participate in on-call coverage as assigned.
• Manage therapy discontinuation and update patient records accordingly.
• Implement clinical management models for specific therapies and disease states.
• Handle intake for new referrals initiated by phone.
• Support pharmacy technicians with claim adjudication and payment accuracy.

Schedule - M-F -10am to 7pm

Pay - The anticipated wage for this position is between $ 59.00 and $60.

Hourly wage depends upon experience, education, geographic location and other factors.

Location - Fort Worth, Texas

Requirements:

-Must have at least 3 years of medication counseling experience

-Must live within one hour of office location- Fort Worth

-Must hold an active Pharmacist License for the state of Texas
